Job: Social Services / Medical Case Manager

We are a 120 bed licensed and Medicare Certified Specialty hospital with a very high acuity patient population. We offer specialized Programs of Excellence for the medically complex including Ventilator Management, Hemodialysis and Wound Care Management.

Trillium currently has openings for a full time Medical Case Manager/ Social Services at our East Valley location.

A qualified candidate will have a minimum of 2 years of hospital or Skilled Nursing Facility Social Services experience. They must have some clinical knowledge and be knowledgeable of Arizona state regulations pertaining to Social Services. This candidate must be willing to serve as a medical Case Manager for a select group of long term care patients.

Mental Health, Drug treatment and Adolescent experience is not appropriate for this position.
